• We are NOT a healthcare provider, medical professional, social worker, financial advisor, attorney, or facility operator.
• We do not provide medical care, prescribe medications, or make medical decisions.
• We do not provide legal advice, financial advice, or tax advice.

#2. No Endorsement of Facilities or Caregivers

• Our inclusion of a facility or caregiver on our platform is not an endorsement.
• We provide information based on data shared with us by facilities and caregivers.
• We do not independently verify all information provided by third parties.
• Families should conduct their own due diligence, including: licensing verification, reference checks, facility visits, and review of state inspection reports (available at azdhs.gov).

#3. Outcomes at Facilities and With Caregivers

For facility placements:

Once a family chooses to engage with a facility, that facility is solely responsible for the quality of care, safety, medical decisions, and outcomes. The facility is also the covered entity under HIPAA for the resident’s protected health information. Find Senior Care Now LLC bears NO liability for events, outcomes, injuries, or losses occurring at third-party facilities after referral. Any disputes related to care quality, safety, or outcomes at a facility should be addressed directly with the facility and reported to appropriate state agencies (Arizona Department of Health Services for facility issues; Arizona Adult Protective Services for elder abuse concerns; AHCCCS for Medicaid-related complaints).

For in-home care services:

While Find Senior Care Now LLC employs caregivers and dispatches them to client homes, families understand that:

• Caregivers are individual human employees who may occasionally be absent due to illness, emergency, or unforeseen circumstances.
• Find Senior Care Now LLC maintains backup caregiver protocols to minimize service interruptions.
• In the rare event of a missed shift, Find Senior Care Now LLC will work to dispatch a backup caregiver and will prorate or refund the missed care per the terms of the Family Service Agreement.
• Find Senior Care Now LLC is not liable for indirect or consequential damages resulting from missed shifts, except in cases of gross negligence on our part.
• Specific terms regarding caregiver shifts, no-shows, and substitutions are governed by the Family Service Agreement signed between the family and Find Senior Care Now LLC.

#4. Owner Disclosure — Affiliated Facility (Meadows Assisted Living)

The owner of Find Senior Care Now LLC also owns and operates Meadows Assisted Living, an Arizona-licensed assisted living facility in Maricopa County.

Financial relationship disclosure

• Find Senior Care Now does NOT collect placement fees from Meadows Assisted Living.
• The owner does receive ongoing rent revenue from residents who choose to move into Meadows Assisted Living.
• This financial relationship is disclosed to all families before any introduction to Meadows is made.
• When Meadows appears in your matched facility options, the affiliation is clearly identified.

Your right to choose freely

• Families are not required to consider Meadows Assisted Living.
• You may request Meadows be excluded from your matches at any time.
• Meadows is evaluated using the same matching criteria as non-affiliated facilities.
• We encourage families to evaluate Meadows the same way they would any other facility — including site visits, reference checks, and reviewing its license and inspection history at azdhs.gov.

#5. No Guarantees

• We do not guarantee placement, specific outcomes, or specific facility/caregiver availability.
• We do not guarantee that any specific facility or caregiver will accept your family.
• Wait times, costs, and availability vary by provider and are subject to change.

• Information on this site is for general informational purposes only.
• It is not a substitute for professional medical advice, diagnosis, or treatment.
• Always consult with qualified healthcare providers for medical decisions.

#7. Financial Information

• Pricing information is provided as a starting reference only.
• Actual costs vary by provider, care level, and individual circumstances.

• Families considering ALTCS, Medicaid, or other public benefits should consult with qualified elder law attorneys or AHCCCS directly.

#10. Caregiver Conduct

• While Find Senior Care Now LLC screens, trains, and employs its in-home caregivers, no screening process is perfect.
• Families are encouraged to stay actively involved in their loved one’s care, observe care being provided, secure valuables, and promptly report any concerns about a caregiver to Find Senior Care Now LLC.
• Find Senior Care Now LLC’s responsibilities regarding caregiver conduct, supervision, and incident handling are governed by the Family Service Agreement.

#11. No Guarantee of Safety or Care Quality

• Find Senior Care Now LLC does not guarantee the safety, quality, or outcomes of care provided by any facility or caregiver.
• For facility placements, we rely on information provided by the facilities themselves and do not independently verify all of it.
• Families should conduct their own due diligence.

#12. Facility Licensing Status

• Find Senior Care Now LLC does not continuously monitor, verify, or guarantee the licensing status, inspection history, or regulatory compliance of any facility.
• A facility’s license may change, lapse, be suspended, or receive citations at any time.
• Families are responsible for verifying a facility’s current license and inspection history directly with the Arizona Department of Health Services (azdhs.gov) before and during any placement.
